To customize your booking form in Reservics, open Settings and go to the Form Builder. There you can keep the built-in system fields (location, date, party, time, name, email, phone, notes) and add your own custom fields, such as dietary requirements or a high-chair request. Each custom field gets a label, type, optional help text, and a required toggle.
Before you start #
- The Form Builder lives in your Reservics admin under Settings.
- The Free plan allows up to 5 custom fields. Pro and Advance plans allow unlimited custom fields.
- Multi-step form layout is a Pro+ feature (Pro and Advance). On the Free plan the form is single-step.
- Changes to the form apply to the storefront booking widget your diners use.
System fields vs custom fields #
System fields are always present on every booking form and cannot be removed. Custom fields are the extra questions you add yourself.

Custom field types #
When you add a custom field, you choose a type that matches the kind of answer you want.

For select and radio fields, you also enter the list of options the diner can pick from.
How to add a custom field (for example, dietary requirements) #
In your Reservics admin, open Settings.
Select the Form Builder tab.
Click to add a new custom field.
Choose a field type. For dietary requirements, Select or Checkbox works well.
Enter a clear label, for example “Dietary requirements or allergies”.
If you picked Select or Radio, add the options (for example: Vegetarian, Vegan, Gluten-free, Nut allergy).
Add help text if you want to guide the diner, for example “Tell us about any allergies so the kitchen can prepare”.
Turn on Required if the diner must answer before they can book.
Save your changes.
Once saved, the new field appears on the storefront booking widget and the diner’s answer is stored with the reservation.
Required fields and help text #
- Required: when on, the diner cannot submit the booking until the field is filled in. Use it for must-have details like allergies. Leave it off for optional preferences.
- Help text: a short hint shown under the field. Use it to explain why you are asking or how to answer. Help text is optional on every field.
Multi-step form layout (Pro+) #
On Pro and Advance plans, you can switch the booking form to a multi-step layout. This breaks a long form into smaller steps so diners are not faced with one big page of questions. On the Free plan the form is single-step. The custom fields and types you can use are the same on every plan; only the layout option and the field count differ.

Where diner answers appear #
After a diner books, their custom field answers are saved on the reservation. You can view and edit them on the Booking detail page in your admin, alongside the date, time, party size, and status.

How do I ask for dietary requirements at booking? #
